This page provides important information for LMCC’s Manhattan Arts Grants recipients about managing their award, along with links to forms required throughout the grant period (January 1 - December 31).

1. Grantee Contract

LMCC will send grantees the Manhattan Arts Grants contract agreement through DocuSign for electronic signature. The document will be sent to the primary contact email address provided in the awarded proposal submitted via Submittable. 

In the contract, grantees will find:

  • Clauses outlining the responsibilities of the grantee and LMCC’s responsibilities as the administrator of the Manhattan Arts Grants
  • The total amount of the grant awarded
  • The funding sources of the grant awarded
  • Crediting language and instructions
  • Information related to Form 1099

Once the contract is signed through DocuSign, a copy will be emailed to the grantee. Grantees should keep this document for future reference.

2. Crediting Language & Logos

All LMCC Grantees must properly credit LMCC and funders on all print and electronic promotional materials related to grant-supported activities.

Funding for LMCC’s Manhattan Arts Grants is provided by the New York State Council on the Arts (NYSCA), the New York City Department of Cultural Affairs (DCLA), the Upper Manhattan Empowerment Zone (UMEZ), and the Howard Gilman Foundation. 

Grantees receiving funding from DCLA, the Howard Gilman Foundation, and/or the Upper Manhattan Empowerment Zone (UMEZ) must include the respective agency logo(s) in all marketing materials. State recipients are not required to include NYSCA’s logo.

Please refer to your grant contract for exact crediting language and instructions.

3. Grantee Event Notification

All LMCC grantees are required to provide information about the public component of their supported projects by submitting an Event Notification form. Creative Engagement & UMEZ Arts Engagement grantees should use the following form:

Keep in mind:

  • Information in the Event Notification Form allows LMCC to coordinate promotion of the awarded project and visits by LMCC staff. 
  • LMCC promotes grantee events through a monthly listing on our website. To be included, the Event Notification Form should be submitted at least a week before the beginning of the following month; late submissions will not be included. Promotion through social media is not guaranteed.
  • LMCC’s monthly newsletter is sent between the first and second week of each month. This may result in some events being missed, but LMCC staff will make every effort to avoid this from happening.

For Creative Learning Grantees ONLY
Creative Learning Grantees are required to notify LMCC staff of their sequential project activities by submitting the Project Activities Form.

4. Changes to Supported Activities

All LMCC grantees are required to share any significant changes in the proposed grant-related activities with LMCC by submitting a Change Request Form.

Common supported changes include:

  • Postponement or cancellation of proposed activities
  • Change(s) in project location of proposed activities
  • Change(s) in the scope of the project (e.g. a reduced number of performances, a reduction in scale, or a significant change in the content of the proposed project)
  • Change(s) in key artistic or administrative project personnel or partner organizations

PLEASE NOTE: The Change Request Form is connected to your grant application in Submittable. Once this form is activated by LMCC staff, you will receive a Submittable notification email with a link to complete the form.

5. Grantee Final Report

All LMCC grantees are required to submit a Final Report form no later than 30 days after the grant-supported activities are fully completed.

The Final Report form will allow LMCC to better understand the success and challenges of the supported project, report to our funders on the impact of these grant awards, and improve our grant programs and services with grantees’ feedback.

PLEASE NOTE: The Final Report Form is connected to your grant application in Submittable. Once this form is activated by LMCC staff, you will receive a Submittable notification email with a link to complete the form.
